Part time/ Full Time permanent positions are available with a range of shifts- mornings/evenings/weekends.
As a Rapid Response Worker, the team will be providing a range of short-term focused social care interventions to facilitate early discharge home from hospital and/or to prevent hospital admissions or residential placement from patients own homes. Which helps to make a true and real difference to people's lives.
You'll be playing a key role in helping vulnerable people live independently in the comfort and security of their own homes.
Minimum 12 months care experience required and working towards a Level 2/3 Health & Social Care qualification or willing to undertake a Level 2/3 HSC qualification.
Full training is provided.
- Rapid Response Worker will be required to:
- Work independently under their own initiative and as part of a team
- Be able to work weekday and weekends (including evenings and occasional unsocial hours)
- Be flexible and adaptable in responding to patients priorities and needs
- Provide a range of personal care services to vulnerable adults, to high quality standards
- Have a working understanding of care escalation pathways, person-centred care, mental capacity principles and safeguarding vulnerable adults.
- Have a full UK driving licence & access to own vehicle
Main Responsibilities:
- Provide personal care and support to a wide client range of needs, illnessess and disabilities.
- To know and understand the care and support need of the client
- Undertake tasks detailed in the clients care and support plan using a person-centred approach
- Encourage independence and motivation of the client
- To provide input into the care and support plans of clients working with the Quality & Compliance team.
- To take full responsibility for the safe handling of property and equipment belonging to the client.
- To maintain good communication and develop effective working relationships with clients and their relatives.
- To maintain detailed accurate records in respect of care, medication and support given and tasks undertaken.
